Homebound older adults suffer an especially high burden of social isolation, loneliness, depression and other psychosocial problems.  But they are also the hardest to reach. This session will review feasible and effective strategies to deliver evidence-based intervention to meet these needs of homebound seniors.

By the end of the sessions, participants will be able to:

  1. List the prevalence and outcomes of social isolation, loneliness, depression and other psychosocial problem in homebound older adults.
  2. Describe potential strategies for delivering home-based service.
  3. Identify evidence-based interventions that target psychosocial problems in homebound older adults.
